death immediately

When I leave 'Barren' downward to a new place, my hero death immediately, whats wrong? Tell me plz!

This is unprecedented, so more information would be helpful. Where exactly do you die, do you die momentarily or do you lose HP and then die, does it happen across games or just this one game, etc.

Could it be that you're just dying to dark eidolon levi? Much like how a lot of new players "instantly die" to normal eidolon levi when he spawns.
